    K21 Health Foundation exists for the benefit of Kosciusko County citizens to ensure health care services are provided, and to advance prevention and healthy living. This will be accomplished by identifying health needs in our community, and maintaining an endowment so funding is available, through investments and grants, for those needs.

    How to Apply

    Applications are accepted quarterly with deadlines on February 1, May 1, August 1, and October 31 at 5:00 pm. Organizations must be non-profit agencies with current IRS tax-exempt status, school systems serving Kosciusko County students, or governmental agencies serving Kosciusko County residents. Projects must benefit Kosciusko County residents and address health and wellness issues.
